Hammad Qureshi
MD
Dr. Qureshi is a Family Physician board certified by ABFM and has been serving the Galveston County area for the last 8 years at UTMB.
He served as a Clinical Assistant Professor at UTMB where he trained medical students and Family Medicine residents over the years. While at UTMB he received Resident Teacher Award in 2016 and Faculty of the Year Award in 2017.
His passion lies in physical and mental wellness. He is an avid runner and competes in races throughout Texas having completed two marathons and a 70.3 Ironman. This passion has sparked a desire to educate and motivate his patients to lead a healthier lifestyle. His special interests include overall health, nutrition, chronic disease management, sports medicine and weight loss.

With over twenty years of health care experience, Stella possesses extensive knowledge in patient care, patient education, and crisis management. Stella has a unique way of connecting with patients to help them achieve their health and wellness goals, as well as a passion for helping patients identify ways to help them achieve long term health goals. Stella has lived in the Tomball area for the last twenty years and is the mom of three sons. She enjoys traveling, taking long walks, and watching football games with her sons.

As a dedicated Physician Assistant with Creekside Family Practice, Heather Cruz, PA-C is committed to improving the health of patients. Heather’s practice philosophy centers on educating patients so they can participate in their own care and providing them with the tools they need to improve their health. Heather has a special interest in preventative medicine and geriatrics.
Heather is a board-certified Physician Assistant, certified by the National Commission on Certification of Physician Assistants and by the Texas Medical Board. Heather earned her master’s degree in Physician Assistant studies at Baylor College of Medicine, completing her training in the heart of the Texas Medical Center.
